Getting to the heart of the problem
Recognizing the root cause of a problem is the crucial prerequisite for fixing it. At the same time, we eliminate the unwanted consequences of the issue at hand. It is essential to focus on the top cause of the problem, often referred to as the "root cause". This is because if we focus on causes that are not root causes, we cannot solve the problem entirely and risk repeating the undesirable situation.
I'm sure you know children's inquisitive questions when they want to discover why and how things work. The methodology is based on chaining the "Why?" questions five times in a row. One why usually triggers another, which in turn again starts another one. Practice and experience have shown that repeating the five whys is sufficient to identify the underlying problem.
